Medical Record Information System at UPT Puskesmas Kotabaru Seberida

Abstract
The development of information technology has had a significant impact on various sectors, including the health sector. One of the main challenges at the UPT Puskesmas Kotabaru Seberida is the use of a manual recording system for the management of medical record data and drug registration. This leads to service delays, the risk of data loss, and low operational efficiency. This research aims to design and build a medical record information system to overcome these problems. The methodology used in the development of the system is the Waterfall model, with system analysis using the PIECES (Performance, Information, Economics, Control, Efficiency, Service) approach. The system design was carried out using the Unified Modeling Language (UML), while the implementation used the PHP programming language and MySQL database. The system is tested through functionality testing and usability testing. The test results show that all system functions are running as needed with a functionality feasibility rate of 100%, and a user satisfaction level (usability) of 90.25%, which is very good. Thus, this information system is able to increase the effectiveness and efficiency of health services in the Puskesmas environment and minimize errors in recording patient data and drug management.
